Please complete and send the form below to place your child's name on the JHYM Retreats Mailing List. Please note that completing and sending this form does not sign your child up for a retreat, it only adds your child's name to our mailing list so that you will receive notices and updates about the JHYM Retreat program. Please submit one form per child.

To use this form to add your child's name to our JHYM mailing list, please follow these steps:

  3. Paste the form into the email that will open addressed to Gretchen.
